Please read the following terms and conditions carefully before accepting. | (1) Entry and participation in Head Start 5K & 10K 2019: (i) By entering this event I declare that I am of age. If I am under 12 years old, I declare that I will only take part in the 5K and will do so with a participating parent or guardian. If I am a parent or guardian completing this form on behalf of a minor I declare that they are of age or agree that they will be accompanied by a participating parent or guardian. (ii) To the extent permitted by law, I accept that the organisers and sponsors of the race, or any of their agents, will not be liable for any loss, damage, action, claim, costs or expenses, which may arise in consequence of my participation of the event. Nothing in these terms of use shall exclude or limit the liability of the organisers or their agents for death or personal injury due to it's negligence or for its fraud. | (2) Medical conditions: (i) I agree that I will not compete in the race unless I am medically fit on the day of the race and that in any case, I will compete at my own risk. I agree that I will note any medical conditions on the reverse of my runner number, and I will make Neurocare aware of any prevailing medical conditions by emailing Failure to note any medical conditions on the reverse of my running number and/or notify Neurocare of any prevailing medical conditions is taken entirely at my own risk. (ii) By entering this event I consent to any medical treatment being administered during Head Start 5K & 10K by official medically-qualified personnel. I agree that the organiser may reasonably prevent me from taking part in the event at any time if the organiser, advised by any medically-qualified personnel, deems my participation in the event to be against the interest of my health and/or safety. | (3) Data Protection and Communication: (i) By entering this event I agree that my details will be added to Neurocare's list of supporters so that we can keep you up-to-date by post with vital information about the event, and how you are making a difference to neuroscience patients in Sheffield. This includes sending you our newsletter and information about fundraising. Any personal details you provide are stored securely. We will never send your details to any third party. (ii) We prefer to use email as our main method of communication with event participants as this is the quickest and cheapest way to let you know important information about the event, allowing us to spend more of your entry fee on changing lives. Please consider choosing to receive important event information by email using the drop down box below. We will only send you information regarding the event itself and will not use your email address for any other purpose. (iii) If at any point and for any reason you would like to change the way you hear from us, how often you hear from us or you would prefer us not to contact you in this way please email or call 0114 226 9678. | (4) Use of your image: (i) I grant my permission for Neurocare and it's sponsors, assigns and licenses to use or authorise others to use photographs, motion pictures, recordings, data or any other record of my participation in this event for any legitimate purpose.
